Founding Engineer — AI Healthcare Startup
We’re building core infrastructure for healthcare operations and focused on skilled nursing facilities (SNFs) where communication gaps directly affect patient care.



We’re live in production, working with real facilities, expanding pilots, and converting early customers. We’ve raised capital and are actively raising our next round.



This is still early. The product is working well, but the system, company, and category are being built at the same time.



We’re looking for a Founding Engineer to work directly with the CEO and turn early traction into a scalable, reliable product.



What You’ll Do
Own and build core systems end-to-end
 Frontend, backend, integrations — you ship across the stack in real production environments
Build for real-world conditions
 Messy EHR data, inconsistent call flows, noisy inputs — systems must hold up under imperfect conditions
Evolve the system as we scale
 Move from early product → reliable, structured, scalable infrastructure
Work with high-stakes data
 Clinical notes, fragmented inputs, incomplete workflows — accuracy and resilience matter
Help define the product
 You’re not just executing — you help decide what gets built and why


What We’re Building
AI-powered phone agent platform for skilled nursing facilities
Deep EHR integration (patients, notes, ADLs, facilities)
Voice-driven workflow automation
Post-call processing pipeline
transcription → intent classification → patient matching → structured records
Clinical language transformation layers
LLM-powered structuring, validation, and storage of clinical data
Multi-tenant application


Tech Stack
Frontend: Vite, React 19, TypeScript, Radix UI, TanStack Query, Clerk
Backend: Node.js, Express
Database: Neon Postgres
Infra: Vercel (frontend), Render (backend)
Voice: Telephony + third-party agent APIs


What We’re Looking For
Strong full-stack engineer who has built and shipped real products
Experience running production systems
High ownership — you take responsibility for outcomes, not tasks
Comfortable operating in early-stage ambiguity


Core experience:

API design and system architecture
Reliable backend systems (webhooks, async pipelines, failure handling)
Working with messy data (normalization, validation, fallback logic)


Bonus:

Early-stage startup experience
Healthcare / HIPAA / PHI exposure
LLM pipelines in production
EHR or healthcare API integrations

A role with rigid specs or clear playbooks
A low-risk, late-stage environment
