Founding Backend Engineer (Remote, US)

Full-time | $180K–$250K + early equity

This is a fantastic opportunity to join an early-stage, venture-backed startup rethinking how real care is delivered online.


This is a hands-on founding backend role. You’ll own core systems end-to-end, shape product direction, and grow into technical leadership as the team scales. If you like solving high-leverage problems at the intersection of product, infrastructure and regulated systems, this is for you.


What you’ll do

Build and own core backend services powering chat, workflows, billing, and data integrity
Design APIs, schemas, and long-running workflows that must be correct and resilient
Ship quickly and responsibly in a real-world, high-trust environment
Help define what gets built, not just how


What we’re looking for

Senior or Staff backend engineer (5+ years) with startup experience
You must have taken at least one product from idea to launch as an early engineer at a startup (Seed - Series C)
TypeScript & Node.js experience
PostgreSQL experience (or similar)
Experience with LLM inference at scale is desirable
Strong systems, API, and data modeling skills
Comfortable owning critical infrastructure in regulated environments
Thoughtful, detail-oriented, and product-minded
Excited by early-stage pace and meaningful ownership
