CareCar partners with healthcare organizations across the US to enhance outcomes and reduce risk, primarily through non-emergency medical transportation and home health services. Our platform powers scheduling, dispatch, provider management, and benefit administration, using a dynamic marketplace model to connect patients with trusted care providers. CareCar is looking for a Senior Software Engineer with deep Elixir experience to help us scale the core platform behind our non-emergency medical transportation services. You’ll join a small, fast-moving team responsible for building and maintaining the backend systems, APIs, and infrastructure that support our patients, providers, and internal teams. We’re looking for a pragmatic problem-solver who can take ownership, lead technically, and ship high-impact features in a fast-moving SaaS environment. You should thrive in ambiguity, communicate clearly, and enjoy collaborating cross-functionally. This is a remote-first position within a mission-driven startup improving healthcare access nationwide. Our tech stack includes: Backend: Elixir, Phoenix, Absinthe (GraphQL), PostgreSQL (RDS) Frontend: TypeScript, React, Remix Infrastructure: AWS, Terraform, GitHub Actions, OpenTelemetry Data & Analytics: MotherDuck, Hashboard, dbt About this role This role requires a proactive, results-oriented engineer who can take charge and deliver in a fast-paced environment, who has excellent verbal and written communication skills. Additionally, you should be comfortable being a key player in a small team where we often wear multiple hats. The software you design and build will be used to address a large variety of use cases, such as: Empowering patients with varied skills and disabilities to manage their care services schedules Providing real-time updates to our call center staff so that they can better serve our patient population Increase the effectiveness and efficiency of our team of care providers through the use of automation, algorithms, statistical models, and machine learning Integrating external data systems to facilitate data-driven decision-making for our team and our customers Improve the quality of the services we provide to patients by creating voice and text communications systems to connect patients with caregivers What You’ll Do: Build and maintain Elixir services using Phoenix and Absinthe (GraphQL) Design clean, scalable APIs that power our web and mobile applications Collaborate with cross-functional partners to design, scope, and deliver features Provide technical leadership and mentorship to other engineers Improve system performance, reliability, and developer experience Contribute to CI/CD automation and infrastructure-as-code (Terraform, AWS, GitHub Actions) Participate in architectural discussion and technical strategy About you You’ve shipped and supported production software at scale, ideally in a SaaS environment You have significant experience (5+ years) writing backend code, with at least 2+ years in Elixir You’re comfortable designing relational database schemas, writing SQL, and optimizing database performance You lead by example: clean code, thoughtful abstractions, and strong testing practices You’re a strong communicator who values collaboration and feedback You enjoy working in small teams, wearing many hats, and making fast progress You are able to overlap around US Pacific Time business hours. Key meetings are typically scheduled between 8:30am-2:30pm Pacific.
